Terms of Service & Disclaimers
1. Acceptance of Terms
By accessing this website or purchasing any digital product from PatientLead Health LLC ("PatientLead," "we," "us," or "our"), you agree to be bound by these Terms of Service. If you do not agree, do not use this website or purchase our products.
2. Scope of Services
PatientLead Health provides condition-specific document products sold as one-time digital downloads. These currently include:
- Condition Packet — a set of plain-language documents for a specific condition: appointment preparation, red-flag guidance, a diagnostic roadmap, and an advocate-navigation booklet.
- Medical Necessity Evidence Dossier — a standalone document compiling the published evidence relevant to establishing medical necessity for a specific condition.
These materials offer education, decision frameworks, documentation guidance, and system-navigation support. Every claim in them is traceable to a published source. We do not provide emergency services. If you believe you have an emergency, call 911 or go to the nearest emergency department.
3. No Medical Advice
Information on this website and within our products is for educational purposes and general support. It is not medical advice, diagnosis, or treatment, and it is not a substitute for care from a licensed clinician. Our products organize publicly available, source-traceable information into structured documents. All clinical decisions remain between you and your licensed healthcare professionals.
4. No Legal Advice
PatientLead Health does not provide legal advice or legal representation. Our products organize published information and do not constitute legal counsel. If you need legal guidance, consult a licensed attorney in your jurisdiction.
5. No Provider-Patient Relationship
Use of this website or our products does not create a provider-patient relationship. Any clinical decisions remain between you and your licensed healthcare professionals.
6. No Guarantee of Outcomes
We do not guarantee that our products will result in any specific outcome, including but not limited to a diagnosis, a change in medical care, insurance approval, or denial reversal. Clinical outcomes are determined by your healthcare providers. Coverage outcomes are determined solely by your insurer. Any individual experiences described do not guarantee results; outcomes vary based on individual circumstances, provider practices, and system constraints.
7. User Responsibilities
You are responsible for:
- The accuracy of information you provide and the decisions you make based on our products or materials
- Reviewing all materials with your healthcare provider before using them in clinical settings
- Verifying any factual claims, clinical information, or insurer-specific details before relying on them
- Using our products in good faith and not for unlawful purposes
- Complying with applicable laws and regulations in your jurisdiction
8. Sourcing and Accuracy
Our products are built so that each claim is traceable to a published source, with a "current as of" date. Published evidence changes over time. We make reasonable efforts to keep materials accurate and current, but we do not guarantee that the information is complete or current at any given time. Review all content carefully before relying on it.
9. Payment, Delivery, and Refunds
Our products are sold on a one-time, pay-per-purchase basis. The current price is shown at the point of purchase. Payment is processed through Stripe at the time of purchase.
Products are digital and are delivered as downloadable files immediately after payment via a time-limited download link. Because the products are digital and access is granted immediately, all sales are final and non-refundable once the download has been accessed, except where we fail to deliver the purchased files due to a technical error on our end. For questions about your order, billing, or a delivery problem, contact us using the information below.
10. Intellectual Property
All content, design, and documentation on this website and within our products is owned by PatientLead Health LLC or licensed to us, except for content you provide. Products you purchase are licensed for your personal use within your own care and the care of those you support directly. You may not reproduce, distribute, resell, or republish the materials for commercial purposes without our written permission.
11. Limitation of Liability
To the maximum extent permitted by law, PatientLead Health LLC shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits, revenues, data, or goodwill, resulting from: your use or inability to use our products; any errors or omissions in the materials; clinical decisions made using information from our products; coverage decisions made based on the materials; or any other matter relating to our products. Our total liability for any claim arising from or related to these terms or our products shall not exceed the amount you paid to us for the product giving rise to the claim.
12. Governing Law
These Terms are governed by and construed in accordance with the laws of the Commonwealth of Massachusetts, without regard to conflict-of-law rules. Any disputes arising from these terms or your use of our products shall be resolved in the state or federal courts located in Massachusetts.
13. Modifications
We may modify these Terms of Service at any time. We will post the updated version on this page with a revised effective date. Your continued use of this website or our products after changes are posted constitutes acceptance of the updated terms.
